GROUPCARE scheme application form

This form is used when practices would like to apply to become a GROUPCARE scheme. Practices where two or more eligible GPs¹ are members of the MDU can set up a GROUPCARE scheme. It is completely free to set up. The information provided on the form will determine which tier of GROUPCARE benefits are available to the practice.

Please ensure you include the names of all of the practice nurses, nurse practitioners and nurses with an extended role you need to indemnify.

GROUPCARE application form for nurses and other healthcare professionals

This form is used when nurses or other healthcare professionals would like to apply for MDU membership. This application form could be used for:

• new members of staff joining the practice
• existing staff who have not joined the MDU previously
• staff returning to the practice after being removed from the scheme.

GROUPCARE consent form

Individual members of the GROUPCARE scheme can give authority to a named individual or position within the practice to access their membership record; this can be of benefit with the management of your scheme.

The level of authority can be to discuss and amend a membership record or just to discuss.

GROUPCARE practice nurse questionnaire

This form is used when a nurse who is an existing MDU member changes duties, for example when a nurse takes an extended role or becomes a nurse practitioner.

Nurse practitioners and nurses in an extended role may undertake certain duties that require a subscription to be paid. The assessment of the information provided on the questionnaire helps us to determine if this is the case.
